A duodenal ulcer is a common type of peptic ulcer—an open sore that forms in the lining of the duodenum, which is the first part of the small intestine just beyond the stomach. They are primarily caused by Helicobacter pylori (H. pylori) bacteria or the long-term use of pain relievers like NSAIDs.

Symptoms

Unlike stomach ulcers, where eating often worsens the pain, duodenal ulcer pain classically presents with the following pattern:

  • Burning Abdominal Pain: A gnawing feeling in the upper belly, typically occurring 1 to 3 hours after eating a meal.
  • Nighttime Pain: The pain often wakes you up in the middle of the night because the stomach is empty and acid levels are high.
  • Pain Relief: The burning sensation temporarily subsides after eating or taking an antacid, only to return later.
  • Other Signs: Nausea, bloating, and poor appetite.

Common Causes

  • H. pylori Infection: This bacterial infection damages the protective mucus layer of the digestive tract, allowing stomach acid to cause inflammation and sores.
  • NSAIDs: Long-term use of medications like ibuprofen (Advil), naproxen (Aleve), or aspirin inhibits the body's natural production of the prostaglandins that protect the intestinal lining.
  • Lifestyle Adjustments: Quitting smoking, avoiding excessive alcohol, and managing stress.
