Introduction

Uttarbasti is a specialized Panchakarma procedure in which medicated oil or ghee is administered through the urinary passage in males. It is particularly useful in disorders of the urogenital and reproductive system. In male infertility, Uttarbasti is one of the most important therapeutic procedures for conditions related to Shukra dhatu (semen), Shukra-vaha srotas (seminal channels) and genito-urinary tract disorders.

Classical Ayurvedic texts such as Charaka Samhita, Sushruta Samhita and Yogaratnakara have mentioned the use of Uttarbasti for reproductive and urinary conditions.

Purposes of Uttarbasti in Male Infertility

Uttarbasti purifies and nourishes Shukra dhatu, clears blockages in genital channels, balances vata dosha in pelvic organs, stimulates semen production and improves sperm quality. It also reduces urogenital tract infections which may lead to infertility.

Indications

  • Uttarbasti is indicated in the following male conditions: -
  • Shukra dushti (low sperm count or poor motility)
  • Klaibya (erectile dysfunction)
  • Alpa shukra or azoospermia
  • Urethral or bladder calculi
  • Granthi or obstruction in Shukra-vaha srotas such as prostate enlargement or seminal vesicle blockage
  • Chronic urinary tract infections and prostatitis
  • Premature ejaculation or painful ejaculation

Contraindications

  • Uttarbasti should not be administered in the following conditions :-
  • Acute urinary tract infection
  • Active sexually transmitted infection such as gonorrhoea or chlamydia
  • Acute fever or systemic illness
  • Known urethral injury or untreated stricture
  • Severe haematuria
  • Uncontrolled diabetes with risk of infection
  • Recent instrumentation or surgery of the urinary tract
  • Severe local inflammation or trauma
  • Active prostatitis with fever

A urine examination and culture should be done before the procedure to rule out infection.

Mode of Action

Medicated oil provides local action by lubricating, nourishing and reducing inflammation in the urethra, prostate and seminal vesicles. Absorption through mucosa balances vata and pitta dosha and stimulates Shukra dhatu. It also flushes micro-obstructions, helps eliminate infective elements and promotes sperm production and seminal fluid quality.
