Introduction
Uttarbasti (literally “administration through the upper route”) is a specialized Panchakarma procedure described by Ācharyas of Ayurveda. It involves the administration of medicated oil, ghee, or decoction through the urogenital tract — specifically via the vaginal route in females and via urethra in males. In females, Uttarbasti is regarded as one of the most effective treatments for gynecological and urinary disorders. It has both therapeutic and preventive value, especially in reproductive health.
Indications of Uttarbasti in Females
Classical texts describe a wide range of indications
1. Vandhyatva (Female Infertility) – Helps in cleansing the uterus, correcting endometrial receptivity, and improving ovulation and conception chances.
2. Artava Dushti (Menstrual Disorders) – Dysmenorrhea, oligomenorrhea, scanty menstruation, irregular cycles.
3. Yoniroga (Diseases of Female Genital Tract) – Cervicitis, vaginitis, chronic pelvic infections, leucorrhea.
4. Garbhashaya Shotha (Uterine Inflammation) – Chronic endometritis, pelvic inflammatory disease.
5. Retention Disorders – Difficulty or painful micturition, recurrent urinary tract infections.
6. Garbhashaya Vikara (Uterine Disorders) – Small fibroids, mild endometrial hyperplasia (supportive management).
7. Sootika Vikara (Postpartum Disorders) – Helps in restoring uterine tone and eliminating vitiated doshas after delivery (only under strict supervision and after lochial discharge stops).
8. Apana Vata Disorders – Generalized pelvic floor dysfunction, chronic pelvic pain.
Contraindications of Uttarbasti in Females
The procedure should not be administered in the following conditions:
1. Pregnancy – Contraindicated throughout pregnancy as it may disturb the fetus.
2. Menstrual Period – Not to be performed during active menstruation.
3. Acute Pelvic Infections – Active pelvic inflammatory disease, cervicitis, vaginitis with severe infection.
4. Active Urinary Tract Infections – Burning micturition, fever with chills.
5. Genital Malignancies – Cervical or uterine carcinoma.
6. Severe Vaginal/Perineal Trauma – Post recent surgery or injury.
7. Severe Debility or Unstable Vital Parameters – Profound anemia, uncontrolled diabetes, or systemic illnesses.
8. Known Allergy to the Medicated Substance – Hypersensitivity reactions to the drug to be used.
Mode of Action (Ayurvedic Perspective)
Dosha Shamana: The medicated oil/decoction pacifies aggravated Vata and Kapha doshas locally.
Shodhana and Snehana: Cleanses the reproductive tract, lubricates, and nourishes endometrium and vaginal mucosa.
Strotoshodhana: Opens blocked channels (srotas) and improves circulation and nerve supply to pelvic organs.
Garbhashaya Shuddhi: Prepares uterus for implantation and conception.
