Definition:

Avascular Necrosis (AVN) of the hip is a Vata-pradhana disorder in which continued depletion of body tissues (Dhatu Kshaya) leads to further aggravation and vruddhi of Vata Dosha. The aggravated Vata causes Srotorodha of Asthi-Majjavaha Srotas, resulting in Asthi and Majja Dhatu Kshaya and failure of proper nourishment to the femoral head. This pathological process ultimately manifests as progressive pain, stiffness, restricted movements, weakness, and degenerative destruction of the hip joint.

Ayurvedic Causes

  1. Vata Prakopa
  2. Asthi & Majja Dhatu Kshaya
  3. Srotorodha (Asthi–Majjavaha Srotas)
  4. Abhighata

Modern Causes

  1. Long-term corticosteroid therapy
  2. Chronic alcohol consumption
  3. Trauma / fracture of femoral neck

Symptoms:

  1. Sandhishoola – Pain in the hip joint →  Deep groin/hip pain, aggravated by weight-bearing
  2. Sandhigraha / Stiffness – Restricted hip movement → Reduced range of motion, limping
  3. Asthi Kshaya Lakshana – Weakness or thinning of bones → Femoral head collapse, osteoporosis-like changes
  4. Gati Bhramsa / Chalapravahta – Difficulty walking →  Antalgic gait, limping
  5. Shoola Vriddhi with Vata Prakopa – Pain increases with activity →  Pain worsens on walking or climbing stairs

  1. Basti Chikitsa (Pradhana Karma)
  2. Snehana
  3. Swedana
  4. Rasayana Chikitsa
  5. Shamana Aushadhi Prayoga
